We are so delighted you are considering applying to Collegiate’s Middle School for your son. Middle School, grades 5, 6, 7, and 8, represent unique non-entry points and limited openings are available through attrition. Please see below the steps required for completing your application for admission. Applicants to Grades 5, 6, 7, and 8 can begin the application process and we will be in touch if any openings occur and if we are able to offer the complete application process.

Application Checklist

Step 1 – Submit Your Application, Optional Photo, and Applicant’s Work Sample

Please submit your application, optional photo, and child’s work sample by November 15, 2024, and keep a copy for your files. We would very much appreciate an authentic piece of work by your child. It could be a self portrait, a poem, a writing sample, a school assignment that your child is proud of, or a photograph of your child in action pursuing his passions and interests. The work sample should be an image or text document. Please note that we are not able to accept videos, our file formatting will not support any videos.

Step 2 – Testing

Step 3 – School Report and Recommendations

Information is available via the application portal.

School Report

Please request an official transcript from your child’s current school. In order to help us know the student, please provide our office with parent copies of the two previous years' report cards if they are not included in the transcript. The School Report must be received no later than January 6, 2025, in order for your application to be complete.

Two Recommendations

The school needs two current recommendations for your son, one from an English teacher and one from a math teacher by January 6, 2025 in order for your application to be complete.

Step 4 – Required In-Person Applicant Visit and Virtual Parent Interview

The Office of Admissions and Enrollment will contact you if we can schedule an applicant visit and parent interview appointments.

Optional in-person Parent Tours: Parents/guardians who have submitted applications for grades 5, 6, 7, 8, are invited to join us for an optional group tour. Visits are offered by appointment only. This is a facility tour that will provide you with an opportunity to see how a boy grows up in a K-12 school, under one roof. Please allow approximately one hour for your visit.
